For which type of pain is OTFC NOT suitable?

OTFC, or On-Table Fentanyl Citrate, is a form of pain management that is effective in providing analgesia for various types of pain; however, it is not suitable for all scenarios. The use of OTFC is particularly indicated for moderate to severe pain, but certain conditions can make its use inappropriate.

In cases of severe pain in shock, the administration of OTFC may not be the best choice because the patient’s systemic condition can compromise their response to opioids. Shock can lead to alterations in circulation and metabolism, which may result in decreased effectiveness of the medication. It is essential to consider that shock can also cause a decreased level of consciousness, potentially leading to respiratory depression, a significant concern when prescribing opioid medications.

In contrast, OTFC can effectively address moderate pain in non-combat situations and mild pain management. Although moderate pain with respiratory distress is challenging, careful monitoring can still allow for OTFC use, depending on the clinical judgment of the provider. Therefore, OTFC is not suitable for patients experiencing severe pain in shock due to the critical balance required in their treatment.
